To stop the spread of a dangerous virus, the government quarantines all infected citizens on the island of Sinrakdo, confining them to a sanatorium for thirty days of treatment. Four years later, a cure has been found, but a small number of patients still remain on the island. Doctor Seo Jeong-hun is dispatched there as a volunteer physician, where he meets a peculiar nurse and unsettling patients. Nurse Hong shadows him constantly and offers eerie prayers before a grotesque Buddha statue every night, while a hostile patient named Ju-cheol warns him that something monstrous roams the hallways at three in the morning. Jeong-hun tries to brush it all off — until one late night, he stumbles upon a secret the patients have been hiding, and a series of disturbing events begins to unfold.
